It indicates that the chain slack is already significant enough to rattle on a cold start.
Other warning signs include the check engine light illuminating with camshaft position sensor fault codes, rough idle, reduced power, and in advanced cases, visible timing marks that have shifted from their correct position when checked by a mechanic.
